What is Needed to Proceed

DOWNLOAD BIRTH FORM or DOWNLOAD DEATH FORM, complete and SIGN form.

Texas residents can only order:
- Their Own Birth Records; or
- The birth records of immediate family members.
(Qualified applicants include by blood or legal marriage: grandparents, parents, children, brothers and sisters.)

Non-Texas residents can only order:
- Their Own Records; or
- the Records of Their Children if They Are Listed as Parents on the Record.

Proof of Identity

You will need one of the following documents, and it must contain the applicant's name and signature and/or an identifiable photo of the applicant. The document must be valid and current.

- U.S.-Issued Driver’s License
- U.S. State-Issued ID
- U.S. Military ID
- Permanent Resident Card or Green Card
- U.S. Passport
- Visa
